Specimen Requirements
Acceptable: Plasma - collected in Lithium heparin gel tube (Light green top)
Separate plasma/serum from cells by centrifugation within 2 hours of collection.
Minimum volume: 0.25 mL
Stability: 3 days at 2-8°C
Reference Values
Age <50 years: 0-450 pg/ml
Age 50-75 years: 0-900 pg/ml
Age>75 years: 0-1800 pg/ml
· In patients with dyspnea, a proBNP level < 300 pg/ml effectively rules out acute CHF with almost 99% negative predictive value.
· Knowledge of each individual patient’s pro-BNP “baseline” level may be more useful than trying to apply similar cut-offs for every patient.
· Marked elevations in NT-pro-BNP may be observed in states other than left ventricular congestive failure, including: acute coronary syndrome; right heart strain/failure (including PE); critical illness; renal failure; as well as advancing age. Falsely low proBNP in CHF patients may be observed with obesity.
